Instructions for completion of the Cattell sabbatical award application.

1. Complete the following application form (download application in Word by clicking here). We suggest using our headings and spacing the application as needed to complete each item. DO NOT SUBMIT YOUR VITA in addition to or in lieu of any portion of this form.

2. Recommendation letters: In support of your application we ask that you solicit letters from at least 2 referees. Send them a copy of the memorandum for referees http://www.cattell.duke.edu/cattref.html. Please ask your recommenders to please mail their letters to us by December 1. Members of the Cattell Fund Board of Trustees are ineligible to serve as references.

3. Institutional Certification: Please have your department chair or dean complete the institutional certification http://www.cattell.duke.edu/cattcert.html and mail this form along with your application.

4. Mail 5 copies of this application and one copy of the institutional certification by December 1 to:

Plan for sabbatical period: (On separate sheets at the end of this application describe in not more than 3 double-spaced pages (excluding references), what you plan to do during the fellowship period in terms of study, research, writing, or other activity that will contribute to psychology or improve your effectiveness as a psychologist. Indicate where you plan to spend the fellowship period. Explain what an additional semester will permit you to undertake that could not be done in a one-semester sabbatical.)

17. Professional references. (Provide the names and mailing addresses (including email addresses) of two professional persons whom you have asked to provide an evaluation of your work. Note that you are responsible for making sure that your evaluators send their letters to us by the December 1 deadline.)
